Good Faith Estimate Notice (No Surprises Act) ‍

Oliva Wellness PLLC is a private-pay psychotherapy practice serving clients throughout North Carolina. Under the No Surprises Act, individuals who are uninsured or choose not to use insurance have the right to receive a Good Faith Estimate of the expected cost of their health care services, including psychotherapy services, before receiving services. A Good Faith Estimate is not a contract for services, and you are not obligated to schedule or receive services at Oliva Wellness PLLC.

At Oliva Wellness PLLC, the out-of-pocket fees for psychotherapy services are $200 for an initial intake appointment and $175 for each subsequent individual therapy session. The frequency and duration of psychotherapy vary based on each client's individual needs, treatment goals, and clinical recommendations. Many clients initially attend sessions weekly or every other week, although treatment recommendations may change over time. Because every treatment plan is individualized, the total cost of services will vary from person to person.

Any Good Faith Estimate you receive is based on the information available when it is provided. If your treatment changes, an updated Good Faith Estimate may be provided.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you may have the right to dispute the bill through the federal patient-provider dispute resolution process.
